We exist as part of the core curriculum in order to serve the masses, individuals, groups, public speaking needs of all areas of study in academe. In May 2000, following local consultations, the former Calderdale and Kirklees Health Authority introduced an out-ofhours protocol for palliative care in the community. The protocol provides guidance and service access information for practitioners, while introducing several new developments in local out-of-hours palliative care services. The health authority organized the protocol around four priority areas: G Communication G Carer support G Specialist medical advice G Drugs and equipment. Communication As part of the protocol, the health authority introduced an out-of-hours handover form for use with all palliative care patients. The practitioners can use the form to record details of the patient's condition and treatment and to set down plans for out-of-hours care, including advice on anticipated problems. Who developed headache after 50 years. Thus, in the Indian population also, migraine is a disease of the young. It has been observed that the prevalence of migraine increases at the time of puberty without significant differences between the genders, with a subsequent increase in prevalence in females after puberty.5, 6 The classification according to the IHS criteria revealed that majority of the patients suffered from migraine without aura with a prevalence of 83.8%. Migraine with aura is much less prevalent while complications due to migraine are rare. Migraine without aura is the commonest presentation. 7 Certain rare symptoms like presence of autonomic symptoms like sweating, gastrointestinal complaints like cramps and borborygmi and vertigo have rarely been documented before. Irrespective of its genetic basis, the incidence and severity of the headache is modified by environmental factors. The most common precipitants in this study were travel, hunger, mental tension, lack of sleep, watching television and various odor producing substances ranging from perfumes and incense to petrol, cigarette smoke and body odor. An interesting observation was that, certain food items like fish, cabbage, red beans and cola drinks and weather conditions like cold air, humidity and even taking a bath could precipitate or aggravate migraine. Such observations have been rarely reported. Certain environmental factors associated with migraine include long hours of television, strong light, working on the computer, eating citrus fruits and being unhappy.5 An association has been observed between migraine and allergic and psychiatric disorders. Such an association has been found previously in migraineurs with significantly more asthma and chronic musculoskeletal pain compared to non- migraineurs 5% versus 3.2% and 39% versus 25%, respectively ; .8 The health related quality of life HRQOL ; using RAND-36 showed a significant decrease with an increasing frequency of migraine attacks without any difference between migraine subtypes. 8 Mental tension and depression, generally considered to be etiologically related to the development of tension-type headache, was observed to precipitate migraine attacks. Though, we have not assessed the presence of depression and anxiety due to migraine with standardized rating scales as that was not the objective of the study, depression was found in 7.5% patients. However, anxiety was noted at the time of visit in 133 patients 67% ; in this study. Patients suffering from migraine are generally underdiagnosed and inadequately managed. NSAIDs was the drug most commonly prescribed for aborting acute attacks and was effective in 80.9% cases. Mosquito infections were performed as described 2 ; , and the mosquitoes were kept at 19C to allow parasite development until RNA extraction.
The median volume of carbon dioxide administered for patients undergoing automated insufflation was 4.2 L. This volume is approximately twice the 2 L recommended by other authors [2] and greater than the manufacturer's technical recommendation ~3 L ; , albeit smaller than in the abstracted literature mean, 6.3 L [Yee et al., 2002 RSNA meeting] ; . It may at first appear counterintuitive that we found larger insufflator-administered volumes were associated with worse distention. However, such large volumes almost certainly reflect either anal leakage or smallbowel reflux because of an incompetent ileocecal valve, both of which would serve to reduce distention. Given that the maximum volume of carbon dioxide contained in an enema bag is limited to approximately 3 L, the difference in distention between the two insufflation methods may in part be explained by insufficient gas available for the manual method. However, the manual operator needed to administer additional room air in only a single patient in our study. Good evidence exists that, in general, patients prefer CT colonography to both colonoscopy and barium enema [12, 15, 16]. It would be problematic if automated insufflation improved distention at the expense of patient tolerance. Using a validated questionnaire, we found that patient acceptance was similar for both insufflation methods, and, in particular, no significant differences were seen for either bloating or discomfort. Patients were significantly more weary after automated insufflation p 0.03 ; . The reason for this is not clear and, because it was the only difference between the two methods, may likely be explained by a type 1 statistical error because a total of 28 individual comparisons were made. The generally good patient acceptance of both techniques may have been in part due to our use of a spasmolytic. However, interestingly, previous work has not shown any particular effect of hyoscine butylbromide on patient experience with CT colonography [12]. It therefore seems that the use of carbon dioxide rather than air may have contributed to the generally good patient tolerance of both the manual and automated techniques. The operator using the automated insufflator did not report any undue technical difficulties, although previous workers have described problems maintaining steady flow of carbon dioxide in patients with tortuous or fluid-filled colons [16]. However, those comparisons used a laparoscopic insufflator device that was not tailored to colonic insuffla and esmolol.

After this period, the cell suspension was pipetted vigorously several times to disintegrate residual pellets and flakes, and after a further 1-h period of incubation, the mixture was strained through two layers of a 40-, um-mesh nylon cloth. After dilution with the same volume of medium and subsequent passage of the suspension through a loosely packed layer of cotton wool, it was centrifuged for 12 min at 10, 000 x g and 15'C Sorvall RC-2B centrifuge and SS 34 rotor ; . Layers of floating protoplasts were collected and washed three times in the same volume of medium and finally suspended in icecold 0.025 M PIPES [piperazine-N-N'-bis 2-ethanesulfonic acid ; ]-Tris pH 6.5 ; -0.6 M mannitol to give 2 x 109 to 5 x 109 protoplasts per ml. Complications bleb failure Postoperative flattening of the bleb is often associated with failure of the trabeculectomy. Bleb failure in primary open angle glaucoma ranges from 67% to 94% of cases. Bleb encapsulation is a frequent cause of failure that is reported to occur in 13% of patients. It often occurs in the early postoperative period, due to adhesions and scar formation between the episcleral tissue and the Tenon capsule. Several factors have been associated with bleb encapsulation. Patients who had a history of migraine, with or without aura, and experienced between one and six attacks per month of severe moderately severe migraine during the previous 12 months were entered in the nasal spray studies. The following categories were not selected for the clinical studies: age 18 years or 65 years, history of non-migraine headaches ie. tension type headache ; , cardiovascular disease ie. ischaemic heart disease, coronary vasospasm, atherosclerotic disease, diastolic blood pressure 95mmHg and systolic blood pressure 160 mmHg ; , pregnancy and breast feeding, females unless adequate contraception was employed, concurrent medication ergotamine or dihydroergotamine, monoamine oxidase inhibitors, selective serotonin reuptake inhibitors, or lithium ; , history of substance abuse alcohol, opiates or ergotamine ; , hypersensitivity or prior exposure to sumatriptan nasal spray or intranasal dihydroergotamine. Specific studies involving asthmatic patients have not been conducted, however a few asthmatic patients were included in the clinical trials. Clinical Studies Conducted In The Adolescent Population Table 4 shows 60 and 120 minute efficacy results for sumatriptan nasal spray 10 mg and 20 mg in a placebo-controlled study in adolescent migraineurs experiencing moderate or severe migraine pain and ergotamine.
