A procedure as open surgery and as safe as laparoscopic surgery." Investigators are exploring various approaches for NOTES--transrectal, transvaginal, and even transesophageal, for instance--but most research to date has been about transgastric surgery. Within that realm, most progress has focused on affirming the general feasibility of transgastric surgery--addressing the "proof of concept"--as well as the ability to close holes with few or no adhesions and without infection. One major application of NOTES may be surgery in the obese patient. Not only could the approach make various operations technically easier in these patients, but it also could offer a natural platform for bariatric surgery, Dr. Kalloo said. Gastric reduction surgery is one of the procedures that Dr. Kalloo, colleague Dr. Sergey Kantsevoy, and others at Johns Hopkins have performed since their first NOTES experience with peroral transgastric peritoneoscopy. Among the others: liver biopsy, gastrojejunostomy, splenectomy, and herniorrhaphy. NOTES was one of the areas highlighted as one of future growth for gastroenterologists by the Future Trends Committee of the American Gastroenterological Association. In a report published last year, the committee said that NOTES may forge a new subspecialty with gastroenterology and surgery, requiring "radically different training" Gastroenterology 2006; 131: 1287-312 ; . Indeed, Dr. Hawes said, some physicians in the future will choose to become "interventional digestivists" who dedicate themselves to NOTES and other more sophisticated endoscopic procedures, such as endoscopic mucosal resection, endoscopic submucosal dissection, and various endoluminal procedures. These specialists will ultimately require "hybrid" training that meshes advanced flexible endoscopy with laparoscopic surgery. "This is still a ways off . it will have to evolve, " said Dr. Hawes, whose own NOTES research is focused on adhesion formation. Just as training models will have to evolve, so will instrumentation. Dr. Hawes and Dr. Kalloo are involved in an Austin, Tex.based company called Apollo Endosurgery, which aims to develop and commercialize new endoscopes for NOTES. Dr. Amitabh Chak, a gastroenterologist who is investigating NOTES at Case.
Present near each opening of the channel. Nuclear Magnetic Resonance measurements reported by Urry et al. 1982 ; indicate that these binding sites are formed by carbonyl oxygens of the peptide backbone and that the carbonyls of Trp- 11 and Trp-13 are most perturbed by ion binding. It is therefore likely that atypical channels result from a conformational change in the vicinity of these carbonyls. The Eisenman theory for ion selectivity Eisenman, 1983 ; seems to describe the pattern of binding strength changes in minis. According to this electrostatic theory, increased binding site field strength should yield a greater increase in site affinity for Na + than Cs'. We found that the sodium conductance is reduced more than the cesium conductance in 90% of the minis we observed Fig. 6 ; . Thus bi-ionic IV's are consistent with an increased binding affinity at one side of the pore. Single-channel reversal potentials V, ev ; using alkali metal cations were similar for minis and standards. This finding can be used to argue in favor of an increased binding affinity as opposed to an increased barrier height. The energetic consequences of the channel modification are likely to differ for different ions. In the simplest permeation models Independence Principle or singleoccupancy ; differential affinity changes do not affect the reversal potential but differential barrier height changes do Hille, 1975 ; . In multi-occupancy channels like gramicidin, differential affinity changes cause small reversal potential changes Kohler and Heckmann, 1980 ; like those we observed with minis, but not the large changes predicted by differential barrier height changes. Binding affinity at carbonyl oxygens 11 and 13 could be increased by a ; local narrowing of the pore, bringing carbonyl oxygens together in closer proximity b ; increased peptide backbone flexibility allowing local narrowing in response to cation occupancy, c ; increased carbonyl oxygen electronegativity resulting, for example, from changes in side chain electron-withdrawing, or d ; a change in electrostatic potential in the channel produced by distant changes, for example, side-chain dipoles. The increased H + conductance of minis rules out the longrange electrostatic field effect d ; . It also rules out the theory that minis result from the binding of a charged ligand near the channel entrance. An externally induced electric field should affect H + in the same way as any other monovalent cation. The finding that minis have little or no decrease in silver conductance argues in favor of a local change in binding geometry mechanisms a and b rather than c or d ; which does not affect binding of the highly polarizable silver ion, but does increase binding of the less deformable alkali metal cations.2 This conclusion must be.

Human Services Registers Over 100 Residents for the Medicare D Program The deadline for seniors and residents with disabilities to sign up for the Medicare D plan was May 15th. There was a lot of publicity about this program. Coventry Human Services facilitated a workshop at the Senior Club and one at the fall Health Fair with Senator Edith Prague. We sent publicity to the newspapers, handed out information at the senior club meetings and at Orchard Hills, made office appointments and discussed the options by phone to many people in an effort to educate, answer questions and register local seniors and persons with disabilities for the appropriate Medicare D program. Three sessions with counselors from Senior Resources were held. Over one hundred persons were registered through this office. Many were also assisted with the CONNPACE and Title 19 health coverage programs. After May 15th people eligible for Medicare D who did not register in time cannot register until the November 15 December 31 enrollment period. At that time there will be a penalty of one percent per month imposed. Individuals who are just turning 65 or newly eligible for Medicare due to a disability and fennel.

Soundness of their design, and the realistic estimates of cost and schedule. The key features of the technical concept of FAIR can be summarized as follows: i ; highest-intensity, highenergy primary ion beams ranging from the lightest to the heaviest elements, from protons to uranium; ii ; optimized high-intensity secondary beams derived in-flight from the primary beams and ranging from energetic anti-protons to beams of short-lived nuclei near the drip lines; iii ; unique beam compression modes to provide the highest short-pulse beam powers; iv ; broad use of cooler-storage accelerator rings to collect and produce high-precision secondary beams with unparalleled phase space properties and unique in-ring experiment capabilities; v ; a high degree of parallel and independent operation of several ion beams and of antiprotons, which provides for a cost-effective but also synergetic operation of the facility. The facility provides for an order of magnitude increase in beam energy over the present heavy-ion accelerators at the GSI Laboratory, but the most important and unique characteristics are seen in the major steps forward on the intensity and precision frontiers. The technical characteristics of the facility provide for exceptional, and in many respects worldwide unique performance parameters that will allow forefront research programs in a number of areas of science. They address in particular: i ; the investigation of the properties and the role of the strong nuclear ; force in shaping the basic building blocs of the visible world around us and of its role in the evolution of the universe; ii ; tests of symmetries and predictions of the standard models and their possible limitations, such as in quantumelectro-dynamics QED ; in the sector of the electro-weak force, and in quantum-chromo-dynamics QCD ; in the sector of the strong interaction; iii ; the properties of matter in extreme states and under unusual conditions, both at the subatomic, microscopic level as well as at the macroscopic levels of matter, antimatter, and bulk materials; and iv ; various applications of high-intensity, high-quality ion and antiproton beams in research areas underlying, or directly addressing, issues of applied sciences and technology. The different research fields address, first of all, specific key questions in their respective areas of science. But they also intersect in common areas such as, for example, the issue of complexity. Complexity runs as a thread through many of the research programs, from non-perturbative QCD and the nuclear many-body system to biological.

Does one spaying procedure lead to post-spay urinary incontinence in dogs? Drs. Kathleen Linn, Rgine Blanger and Cindy Shmon WCVM ; . Up to per cent of spayed female dogs develop urethral sphincter mechanism incompetence USMI ; after female neutering surgery. While the underlying cause of this long-term complication is still unknown, some clinicians suspect that traction on the uterus during an ovariohysterectomy removal of the ovaries and uterus ; might injure the nerves to the urethral sphincter. During the next 12 months, a WCVM research team will use two female neutering techniques on 30 canine patients admitted to the College's Veterinary Teaching Hospital for elective spays. Half of the dogs will undergo an ovariohysterectomy -- the surgical procedure taught to North American veterinary students. The second group of dogs will undergo an ovariectomy removal of the ovaries ; -- the procedure taught to European veterinary students. Before all of the surgeries, the research team will measure each patient's maximal urethral closure pressure MUCP ; . Clinicians consider this measurement the most important parameter in evaluating urinary incontinence. Twelve months later, researchers will repeat the MUCP measurement during each dog's checkup. Once all of the study's results are evaluated, the research team will determine whether a particular spaying procedure does cause an increase in the incidence of post-spay urinary incontinence among female dogs. Will synthetic corneal implants work in cats and dogs? Drs. Bruce Grahn and Lynne Sandmeyer WCVM ; and Dr. May Griffith University of Ottawa ; . Blinding corneal disease is a common problem in cats and dogs. Veterinary ophthalmologists rely on corneal transplants to treat affected animals, but it's expensive and difficult to maintain corneal banks for animals. As an alternative, two WCVM scientists will work with a researcher at the University of Ottawa to test the use of synthetic corneas that are stronger, more resilient and flexible enough to cover the full range of transplantation needs in companion animals. In the project's first stage, veterinary ophthalmologists will implant synthetic corneas in rabbits, and if these models are successful, then the team will conduct synthetic corneal transplants in five dogs and cats that have been diagnosed with severe corneal disease. Throughout the year after surgery, researchers will regularly assess each patient's recovery and determine whether synthetic corneal transplants are successful in restoring vision without significant ocular complications. The present study is, as far as we know, the first systematic implementation of textual inferences arising from the six types of implicative verbs presented in Figure 1 and their interaction with factive verbs. In this work we have focused on cases where the judgement of whether the author is committed to the truth or the falsity of a complement clause can be made reliably from the sentence in question. Further work is needed at least in the following three areas. Lexicographic gaps. In our classification we only considered simple verbal and adjectival complements. We have yet to study and determine the semantics of complement constructions associated with nominals in collocations such as take the trouble to, have the foresight to, take time to, for which there is virtually no literature. Conversational implicatures. It is well known that constructions such as be able to yield a negative entailment in a negative environment. Ed was not able to open the door entails Ed did not open the door. There is no entailment in the corresponding affirmative sentence. Yet, if the author writes Ed was able to open the door and says nothing to indicate that the door was not opened, the reader is likely to infer, and justifiably so, that Ed opened the door. This kind of conversational implicature is cancelable Grice [6] ; . It is not a contradiction to say Ed was able to open the door but he kept it closed. If a student asks his professor Did you have the time to read my paper? and the professor answers Yes but has not read the paper, the answer can be literally true and very misleading at the same time. 8 Degrees of "factivity". Factive verbs and constructions do not constitute a uniform class. Looking at the pattern of usage of verbs such as mention that, report that, say that, etc. on Google, we observed that in cases such as He did not mention that Coalition allies now plan to leave it was virtually always clear from the context that the author believed the complement to be true. The verb report is similar to mention but there are also cases where .did not report that X was meant to suggest that X is false. On the other hand, .did not deny that X suggests that X is true, whereas nied that X is noncommittal with respect to X and ferret.

Traveller proceed to the latter, in the prosecution of his business he is subject to at once a further impost, which is fixed by the local Authorities at a much higher rate. Provincial licenses represented a sizeable expense for merchants. They were so high that a merchant who wanted to sell in every province had to spend $ 10, 000 annually, a respectable sum of money that only large firms could afford a middle-size industrial company had, roughly, capital assets of $ 100, 000 ; . Fees varied from one place to another; while the fee for trading in the city of Buenos Aires was three pounds and three shillings, in the Northern provinces of Tucumn and Salta it was as much as 70 pounds. It was rational to pay those fees just if the interior provinces demand was worth such an spending. The interior was an attractive market for manufactured goods, both those produced in Buenos Aires and those produced abroad. In the case of foreign firms, this trade was important enough to become the main target of some import houses and to concern the British consul who made a trip to the provinces in 1907 to study his country's commercial activity there. For industrial items produced in the Argentine capital, the interior became increasingly important. In the early 1880s, as a general rule, nascent Buenos Aires industrial firms confined their sales to the city. At the beginning of the twentieth century most of these companies had started promoting sales in two columns A nuestra clientela de la capital A nuestra clientela del interior To our customers of the capital To our customers of the interior ; as trade with the interior became widespread. But for Buenos Aires firms, the interior had an extra appealing which did not correlate with strict economic gains. Buenos Aires industrial firms began a battle against their interior counterparts and succeeded; then, they could enlarge their circle of consumers. Although the interior was important for these companies sales, its significance went beyond making profits. Every corner of the country was worth supplying with local merchandise, regardless of the importance of its demand. Even markets that were left aside by importers were meaningful to Buenos Aires companies. The Provinces of San Luis and Catamarca, to which the British merchants did not bother to send travellers since Ait seems that it has not been worth while, were of interest to the establishments of the capital city. In this effort, the private sector was helped by the state, which built the railroad lines to areas where traffic was not profitable, such as controlled by the Federal Government.
